IBM computer crash - top 5 largest crashes

Air New Zealand got a wakeup call on Sunday when a computer crash grounded their fleet and stranded passengers.
If Rob Fythe email is anything to go by IBM will surely be licking her wounds this week, however to make the boys over at IBM feel better I have put together a list of what I think are some of the biggest "oops" in recent IT history.
Number 5
U.K biggest computer crash in government history – a routine software upgrade caused major problems – a little over 80,000 civil servants were disrupted! - Like the routine software upgrade on the Pukekohe telephone exchange I guess!
Number 4
When software bugs and space rockets combine you can have some very expensive problems! A small software bug destroyed an Ariane 5 rocket in 1996. - Woops
Number 3
In keeping with power problems, in 2003 a massive black out was caused in North America by a race condition in some monitoring software. – Quickly reboot the system I am getting cold!
Number 2
A medical application that was not built within the specifications of the “Three laws of robotics” – and tragically killed 6 people when the radiation machine zapped patients with 100 times the intended dose!
Number 1
The Y2K bug – this sums it up best of what did not happen.
