I now have internet connection w/my Fritz!box.  I also have the Fritz!Box set to a Static IP in the OPNSense firewall DHCP.  I've setup the port forwarding exactly as you described for only the Fritz!box entries (Total 4 entries under Firewall:Port Forward).   My Fritz!fon now only has a busy signal when I attempt a call.  Is there something I'm missing for the Fritz!Box to work as only a VOIP engine?  Here is a screenshot of the configuration on OPNSense.

Also, I read elsewhere in the pfSense forum that another person had to setup two firewalls in order to fake the Fritz!box into having it's Public IP rather than a private one when used under OPNSense.  Do you have a similar setup?

Have you seen this page:

https://doc.pfsense.org/index.php/VoIP_Configuration

On there, the most likely option that could be causing problems is source port rewriting.  Try turning that off.

Have you gone into fritzbox and changed connection type

Internet connection > Operating Mode > Share existing Internet connection in the network (IP client mode)

Attewell;

Success!!  I now have VOIP with the Fritz!box connected behind OPNSense.  The last issue was to disable source port rewriting by setting a Static Port via a Hybrid Outbound NAT rule for UDP to the Fritz!box as you recommended.  Thanks to your hints and direction.

Thank you for your help!!!  As mentioned before, OPNSense is a fork off of pfSense and is a rather good one.  You can apply your pfSense logic to it and make it sing for you.
