That is plenty light. What was the focus distance. You don't need manual focus at macro distance, just when it is within 5-10cm of its MFD. How far away was the flower? Did you try anything else other than the flower.

Esawers you need to understand that the mfd for this lens is a lot further than the mfd of your kit lens. This is NOT a macro lens.

The window on the lens says 1.2m minimum, so I've been standing at least 1.5m back
The cat photo was about 3m back

Does the MFD change when you zoom in?
Standing about 2.5m away from the kids, I couldn't focus at 200mm

There is plenty of light available reading from your exposure settings. Take it to a shop and compare.

Mfd does not change with focal length. It is fixed. There is something wrong. If you got sold knowingly with something wrong ... Then... Scumbag they are!

esawers: It's definetly on 1 shot
works better in good lighting,  hardly works at all in muted light, its a bit intermittent 
I don't mind sending it to Canon (I got it super cheap from a retired photographer) but wondered if I was expecting too much

Ensure the auto focus isn't on the limited 3m setting on the side of the lens. That changes the minimum distance it will auto focus to, to help give quick focus at normal distances.

Thanks! It is on 3m - infinity
The other option is 1.2m to infinity - is that the one to use??

Edit: Obviously it is! Makes more sense. I'll give it a go

Erratic autofocus can be a symptom of poor electrical contact between lens and body, so make sure the electronic contacts are clean.  Some people suggest using an eraser to wipe across the contacts.  Be careful doing this - not to drop debris into the camera body or lens.  "Deoxit" is a contact cleaning system often recommended for cleaning those contacts.  Jaycar stock it - not cheap though.  I initially thought it was probably snake oil, but since buying some and using it - IMO it is extremely good.
Unfortunately the issue you describe can also be a symptom of USM failure - which is expensive to fix.
