Thanks for your question. A domain that is shown as "Premium" has been categories as a premium domain name by the respective registry. Those names have a special price assigned by the registry. If you want to know, we are happy to get in touch with the registry and ask for you. Please note that Premium domain names won´t be covered by our promotion.

Can you explain the free email part? I've got my domain and according to the steps I now need to login to my account, manage my domain, and activate my free email account. Which option is included - they all show a cost?

That's only if you want additional accounts. If you don't add it to the shopping cart (icon to the right), you won't get charged. You get one free email account regardless.

Now I've registered my domain, how do I point it to my home PC/network?
Is it a matter of:
1. Click on "Manage" for my domain
2. Click "Manage Zone"
3. Edit the A record to change it to the IP address of my home network. (I expect that from time to time I'll need to update this when Spark changes my IP address).
4. Should I make "Dynamic DNS" active or inactive? I'm on an ordinary Spark broadband connection (not fixed IP).
And, now that I have my email account, how do I collect mail from there, including by phone and PC? Auto-forward to my existing Gmail address? Or can I use Thunderbird or similar on my home PC to collect it via POP or whatever?

You are welcome. Correct, you need to click on "Manage" > "Manage zone" and there edit the A Record to add your IP address.
You can leave the Dynamic DNS inactive.
Regarding the setup of your mailbox. You can choose any mail client you like. You will find the setup information under the following link: octopoly.com/help/faq/how-can-I-configure-my-email-address-mailbox

With these 'premium' domains, is that a one off purchase price, and future years are at the $38 fee when it is transferred across? Or is the higher premium price a recurring price every year?

Depends on the registry but most premium domains cost the same price for the renewal as well.
