Is there any thing to fix this error on a Telecom R6 phone. I have called the help desk at telecom but they were unable to fix the fault and my brother needs his phone. Do we need to get a new sim or is there something we can do.

Also if we have to replace SIM is it going to cost?

It doesn't sound like an 'error' it sounds like someones put the wrong PIN in too many times. Telecom staff have access to (or access to the team) who can give you your PUK code. If you enter this incorrectly ten times then your SIM is rendered useless, and yes you will probably have to pay for a new one

Decimate is correct that it is not an fault but user error.

On the top of the sim card pack (the bigger card that the little one came stuck in) is the PUK code which should unblock it... Providing the PUK code hasn't already been attempted too many times. If this is the case then you will need to pay to have it replaced which should be either $20 or $30 can't remeber off the top of my head but I think it's the former.
