Recommendations required for a graphics card which runs cool and doesn't need a fan if possible.  Won't be used for gaming, just HD video. I'm currently using an Integrated ATI Radeon HD 4250 but it only does Bob deinterlacing without stuttering, and I would like vector adaptive deinterlacing.

I have a spare PCI-E 2x slot and I may have a 16x slot also free. Which is required?

Anything newer than AMD 54xx or Nvidia GT220 should be capable of vector de-interlacing... and given both of those models are a few years old basically any new card available now should be fine I think.

I wasn't sure how high up the AMD model numbers I would need.  I was using an integrated AMD 3200 and that handled bob without stuttering, and then upgraded the motherboard which has the AMD 4250 and it made no difference.  It still stutters occasionally when set on Adaptive (using MediaPortal LAV filters).  So you say a 54xx will handle it.  Being that it is an AMD processor board I suppose I'll stick with AMD and look around.

I'm using a Asus Radeon HD6450 Silent LP HDMI 2GB which is working very well for me.

Although now I've read around the topic of deinterlacing, I think I probably haven't got it set up the best way!

sapphire radeon 5450 is ~ $50 - runs fanless and has low profile bracket in the box.

They work pretty well and in my case have provided pretty perfect playback (although I do have a GTX 750 under the hood).

They work pretty well and in my case have provided pretty perfect playback (although I do have a GTX 750 under the hood).

I have already implemented everything I can to get more out of the integrated card.  It just doesn't have the grunt to do decent deinterlacing.

I spent some time investigating options last night and found the AMD 5450 is borderline for VA deinterlacing.  It will do it most of the time but in some situations will fail.  If you set the option to Enforce Smooth Video, it disables vector adaptive on this card.

I also found the AMD 6450 with passive cooling will do the job easily and that is what I will likely get.

The Gigabyte GTX 750 I have has dual windforce fans but is near silent as it never heats up enough to get the fans running at more then low speed. I say this as you may find that a non-silent card will actually be no louder in your system given that you probably already have a CPU and power fan. This gives you more options for cards.
