Seeing a few reports of this around the web. If you don't want to upgrade you might want install KB3050267 for Server 2012R2 or KB3050265 for Server 2008R2. Which will add a new option to group policy to block the upgrade.

Computer Configuration > Administrative Templates > Windows Components > Windows Update > “Turn off the upgrade to the latest version of Windows through Windows Update”

Ive seen this, on a laptop.

Turns out, when that user was out of office, he would log in using a 'local' a/c rather than login using his domain login a/c.
So Im guessing, using a local(non domain) login, Winupdates didnt see it as being on a domain.

Had a client caught out, have now hidden the update :)

Applied the gpo fixes but it doesn't stop this appearing when The user goes tonwin updates and this is selected
