In the cars I use the built in Bluetooth which is fine, elsewhere I don't bother with one.

Alpine CDE-134BT bluetooth headunit and my regular cellphone. Works very well, fairly clear at both ends.

A friend has an older Jabra unit and I've been VERY impressed with it. Call quality at both ends is excellent. Pairs to your phone via bluetooth and transmits to the car stereo via FM also meaning you can stream music to the stereo via Bluetooth --> FM, the quality is that of FM so mostly acceptable. The closest equivalent they do appears to be the Jabra "Drive".

I just plug the 3.5mm into the stereo and shout towards the phone.

Bit echoey for the other person but stuff it, if they cant deal with that they can call back later.

My wife used to put her N8 on loudspeaker and that worked pretty well.
