MarkSW: Perhaps it is time that Telecom looked at offering VDSL services to their customers.
MarkSW: The current ADSL/VDSL splitter is approximately 5 weeks old. It was replaced by a technician (at my own cost) together with the wiring when the issue first raised it's head. The cable is CAT6 directly from the splitter to the modem. The splitter is in the Demarc box where the line comes into the house and is attached to the Telecom phone cable.
MarkSW: Prior to the splitter being replaced a CAT6 cable was run directly from the incoming Telecom cable through the window to the modem. This made absolutely no difference to the SNR reading (all other wiring was disconnected so it was basically a "clean ADSL connection.) I went ahead and had it all replaced anyway as I had the tech there at the time and wanted to make doubley sure that the issue was not on my side.

We have a shared driveway down the side of our house with a grey Telecom connection box, which I assume is for the houses in the vacinity. I have had Telecom out twice since moving in to repair (secure the cover) the box, as our neighbours (3 of them) now and again reverse into it and pop the cover off!
StealthyMoose: I switched to the 500gig 6 weeks ago, and until the last 2 weeks hadn't noticed any change.
When downloading I was consistently getting 1.0Mps , but now it has dropped to 420 Kbs (this is the average speed over the period of large download)
I'm less than a km from the exchange in a CBD
Telecom speed test is as follows
Download Speed 3.64Mbs
Upload Speed 0.79Mbs
Ping 67ms
I had requested interleaving be turned off sometime ago prior to changing to the 500gig plan, and it wasn't immediately following the change that the slow down happened.
As of writing this I haven't contacted Telecom, I was hoping to gather some feedback prior so at least if they suggest something I can be somewhat informed as to what it is!
What I find strange is that
a) The speed test is 3 times higher than any actual download speeds I have every experienced
b) My download speeds are very much round fiqures ie 1.0Mbs and 420Kbs (suggesting that these are governed by a predetermined setting???)
sbiddle: DSL speeds are ~80% of the sync rate. With a sync rate like that there is no way you could get 12Mbps.
The question is why your sync rate is as low as it is.

Modem is suffering from extreme case of "restartilitis"
Restarted plenty of times.

Looks to be locked to ADSL1 not 2+ given the sync rate and very high noise margin. What is your DSL connection type set to in the modem?
