Test. When I reply to a topic a duplicate post is created. Happened for each of the posts I made today and yesterday. I will reply to this and see if it also occurs. Chrome on OSX.

I've seen duplicated replies and most of the time they were from mobile users hitting the refresh button or going back before the page appeared for them, in effect submitting the reply again.

Could be something like that. In the two occurrences I most likely hit quote and then logged in. (The quote button is visible prior to login). Trying that now.

Nope, but it's probably something to do with a refresh after being logged out like you mention. Might come back to it later.
