You can create a 'custom trunk' in FreePBX.

Then you can use outbound routing to get the calls you want to go to that 'trunk'.

Ok.. Now this is completely untested but makes sense :P what you can do is edit /etc/asterisk/extension-custom.conf and add something like

[to-external-transfer]
     exten => s,1,Transfer(......)
     exten => s,n,Hangup()

then Under Admin->Custom Destination you can add one new custom destination. the "Custom Destination" field will be "to-external-transfer,s,1" (without quotes)

Then, you would probably set up a an incomming or outgoing route, or maybe a custom trunk which has its destination set to the one you created. (I'm really not sure with FreePBX)

Sorry I can't be much more help.. I haven't got any spare hardware to play with. but I'll let a more qualified person chime in :P
Eventually your dialplan can look something like this

exten => s,1,Answer()
exten => s,n,Playback(pleasewait)
exten => s,n,Wait(3)
exten => s,n,Transfer(02xxxxxxx)
exten => s,n,Hangup()

I did this on mine with Press 1 to leave a message, Press 2 for after hours, etc
