if your other flatmates are willing to move with you, sort out another place to move to, wait for a day when the disruptive flatmate is at work, and all of you move your belongings and things to your new place.
Judging by what you have said, there is no way to change his behavior. Move out and move on!
Don't tell him what you have planned, especially not the address you are moving too.
Would love to be there to see the look on his face when he arrives home to find the place empty and everyone has moved out!
Moving out is the best option if your name is not on the tenancy document.
I had a similar problem a few years ago with the head tenant, she would go crazy and blame us for things like when the washing machine (which was under warranty) packed up and I got the blame for it and when the tech came and looked at it he found there was a faulty pump, replaced it and she shut up.
She was not happy with anything we did in the house like putting the computer in the lounge in the corner beside the tv (and area that was not even being utilized) so we could watch movies off the computer and when I fixed the satellite cable (Telstra had cut it off at the wall) she was thinking we were getting charged for freeview etc. She was not happy when my contracts I was doing finished as she was worried I could not cover the rent if I did not get another contract in a hurry. When someone else bought the place they increased the rent and we knew what each other was paying but not her and we asked what the total rent was before and after and found we were covering her rent of the place in which she said she would pay all of the extra rent (which she should of just done in the first place and we would of not got to that situation).
After a while of this I found a letter to the newest flatmate and she had put in it her complete dislike for myself and another flatmate so we started looking for a new flat. We had not told her of us moving out and while looking for a new flat found our rooms up for rent on trade me and she had not even told us she was showing people our rooms.
We moved out a week later after finding this and it would probably be the best option in your current situation.

Go tell the landlord hopefully she will understand, being a landlord she wouldn't want any heat to come down on her. Hes already punched holes in the wall that's enough for any landlord to sack him.

I would talk to the other flatmates first. No one has mentioned location and price (you say youve got it at a good one). If the other flatmates feel the same, write something they can all sign, so you dont all have to see the landlord. Explain the damage etc done, say that the 4 of you are going to look for another place unless you (the landlord) remove him from the house. Landlords DONT like looking for people for there houses. Empty house = no income. If he is left by himself should the 4 of you treaten to move, chances are he wont be able to afford the rent, his friends are probably like minded, so they would destory the house.
Of course, if the location and price are average. Just move out. Tell the flatmates your going, and should someone/s wanna go as well, all good, if not o well.
ps, if the landlord removes him I would take the day off work that he moves out.......or have that lock on your door already. He wont be too happy and i doubt it will be his stuff he takes it out on...
