Depends how old it is. Early ADSL ones had two pairs (orange/blue), the later ones had 3 pairs of cables (green/orange/blue). The later ones are also rated for VDSL.

it can be

 

earlier ones only did adsl frequencies which ae a lot lower than the VDSL ones so wont work very well for VDSL

Yep, that's an early ADSL one. Fairly straightforward to swap it for a VDSL one in most cases.

So today it's as bad as ever disconnecting every now and then.

 

With new modem, all new cables between modem and the wall, modem and the router, and of course new modem power supply.

 

Any ideas? 

 

This is so random! It works perfectly for a day and then it decides to play up again. Arrrggh!

Sounds like time to report a fault to the ISP.  You've done all you can.  Might as well order VDSL at the same time.
