Tonight there's a movie on TV1. It is rated 18+. Parental control is set to allow only up to M, because we have 2 kids 7 and 9 years old. Obviously there is no lock on analogue FTA channels, but does that mean Sky may not lock FTS channels even though the 18+ rating is shown in the synopsis? We have the small silver box, but presume it is the same on all boxes.

Yes, SKY decoders can't apply parental controls to FTA channels. It has to do with the parental control mechanism being implemented in the CA (conditional access) so it can't work on unencrypted channels.

Fair enough regarding the encryption stream, but the box knows it is an 18+ movie. When changing channels the rating is shown, not in the description but to the right where rating of all other channels/shows is shown. So the box knows the rating, it is just not acting on it as it is implemented at CA level. Sounds like a stupid design (I'm an electronic engineer).
