Is it easy to set up a new account on a second hand TiVo or does the original account have to be deactivated?

I would imagine it would have to be deactivated. From the Mytivo website.

Can I activate my TiVo? media device more than once?

No ? you can only activate your TiVo? media device once.

If you attempt to activate it a second time you will be informed that the TiVo? has already been activated.

Best bet is to email tivo and ask the question.
