Is setting up a twitter business account the same as a personal account? Do you have to identify as a business, pay a fee - ie. do anything different to setting up a personal account?

Exactly the same. Tweetdeck now has features to let other people use the account without sharing the password for it, so you can all set up 2factor to hopefully not get hacked and have racist or otherwise offensive comments posted to it like seems to be a regular occurrence on there for large accounts.
