Some feedback... it doesn't feel particularly well written. The questions could use more work to clarify them - kitchen ritual question is especially vague and varies between meals, plus frustrations should have an other box with a text field I think. Your frustrations are fairly basic and don't appear to have been given much thought. You could give an introduction to say why you're doing it in the survey. You could also work on your grammar and punctuation. Overall the impression is of teenagers project.
