I had that problem too and it turns out that even though I have a RealMe identity that it's not real enough.
I went to the RealMe site and logged in and now they want additional pictures and video of me and it's all now being processed. I guess I'll be able to log in for real once they decide that it's really me.

That's true. Setting up and maintaining RM is a PITA (although having it already set up made the NZTA thing very easy).
It would be interesting to know how many Kiwis actually have RM set up - due to its PITAness I'd guess it's not widespread. If that's correct, take-up of the NZTA app may be limited so long as RM is required.
Right now I can't even get into my RealMe account. Still getting the following when I try to retrieve my account

Site is probably overloaded with 100 people trying to log in due to the new NZTA thing.

Did you have a RealMe verified identity? I don't know if the app makes this clear or not, but just having a RealMe login isn't enough - you need to have a RealMe verified identity.

I've had it long enough that a verified account wasn't even a thing when it was created.
I agree that it needs to be far clearer about the requirements and communicate that it failed due to the account not being verified rather than just a generic message that something went wrong.
How it's written at the moment just makes it seem like you need a RealMe account, including what you need to create this, and it doesn't make it very clear that you need special attributes on the account.

I did the stupid nodding at my iphone thing and go all mine verified a few months ago. they even sent me an email to say so.. no joy on the NZTA app though

Just looked again at the Waka Kotahi web page where this is linked to, and I think the wording should be changed to be clearer. It currently says, "you need a RealMe verified log in." But the correct terminology should be: "you need a RealMe verified identity to log in."
This is partly DIA's fault, since they have done a poor job of differentiating between a "RealMe login" and a "RealMe verified identity".

Since I wasn't aware that there was a difference, I must admit that I would still read that and go yup, I've got a RealMe account. I'm good to go.
It's annoying that there are now different account types, but that needs to be explained, and if you have the wrong type, then this needs to be communicated rather than just .... Whoops, something went wrong, try again.
"Verified" is not a new thing. I have had two passports issued online using RealMe Verified ID. And this app made me check my current status - expires in a month, so I just re-done the online verification.
My daughter ordered a new passport last week (first after turning 18) and there's a box at the bottom to add "Verified" automatically, since they are already sighting the passport. Otherwise for new ones you'd need to go to a Post Shop to complete verification.

You're both right, and I go back to my previous point about partly blaming DIA for not differentiating enough between the two. In my opinion, "RealMe Verified" should be a different product with different branding so you easily differentiate it from the regular RealMe login. If you look at the developer guides, there isn't even a "RealMe Verified" button that DIA provides to differentiate between them. Instead, they recommend using the same button type with some different text: "Share your details with [organisation]". Pretty confusing if you ask me.

Not too confusing - verified means verified. This was pointed out in the OP and has been commented upon in several subsequent posts.

Not sure what the details at https://nzta.govt.nz/about-us/app/ originally said, but if you go there now, under the RealMe heading, it now says:
To access the current version of the NZTA app, you need a RealMe verified log in. Set up or check your RealMe account for verification on the RealMe website.
In future, the app won't require RealMe verification and an alternative log in will be available.
I can't get into my RealMe account. After I log in it asks me for a 6 digit code from my 'authenticator app', but I don't think I have anything like that.
Hopefully passport renewals can be done by some other manual procedure for those of us who don't understand tech stuff.

At some point you enabled Google Authenticator, or Authy for your RealMe account.
