To the Editor:

Charles M. Blow (column, nytimes.com, Feb. 9) describes Donald Trump’s constant need “to grind the opposition underfoot.” As mental health professionals, we share Mr. Blow’s concern.

Silence from the country’s mental health organizations has been due to a self-imposed dictum about evaluating public figures (the American Psychiatric Association’s 1973 Goldwater Rule). But this silence has resulted in a failure to lend our expertise to worried journalists and members of Congress at this critical time. We fear that too much is at stake to be silent any longer.

Mr. Trump’s speech and actions demonstrate an inability to tolerate views different from his own, leading to rage reactions. His words and behavior suggest a profound inability to empathize. Individuals with these traits distort reality to suit their psychological state, attacking facts and those who convey them (journalists, scientists).

In a powerful leader, these attacks are likely to increase, as his personal myth of greatness appears to be confirmed. We believe that the grave emotional instability indicated by Mr. Trump’s speech and actions makes him incapable of serving safely as president.

To the Editor:

Fevered media speculation about Donald Trump’s psychological motivations and psychiatric diagnosis has recently encouraged mental health professionals to disregard the usual ethical constraints against diagnosing public figures at a distance. They have sponsored several petitions and a Feb. 14 letter to The New York Times suggesting that Mr. Trump is incapable, on psychiatric grounds, of serving as president.

Most amateur diagnosticians have mislabeled President Trump with the diagnosis of narcissistic personality disorder. I wrote the criteria that define this disorder, and Mr. Trump doesn’t meet them. He may be a world-class narcissist, but this doesn’t make him mentally ill, because he does not suffer from the distress and impairment required to diagnose mental disorder.

Mr. Trump causes severe distress rather than experiencing it and has been richly rewarded, rather than punished, for his grandiosity, self-absorption and lack of empathy. It is a stigmatizing insult to the mentally ill (who are mostly well behaved and well meaning) to be lumped with Mr. Trump (who is neither).

Bad behavior is rarely a sign of mental illness, and the mentally ill behave badly only rarely. Psychiatric name-calling is a misguided way of countering Mr. Trump’s attack on democracy. He can, and should, be appropriately denounced for his ignorance, incompetence, impulsivity and pursuit of dictatorial powers.

His psychological motivations are too obvious to be interesting, and analyzing them will not halt his headlong power grab. The antidote to a dystopic Trumpean dark age is political, not psychological.

 I don't see him as mentally impaired. He is who he is, always has been like that. He can get away with that if he is the boss, or he has pull when making deals, but as POTUS he is not the boss of the USA, he is the leader of the governing party, and he is not the boss of the globe, he is a peer in the globe. All of which seems lost on him. The Cream will rise to the top and the lead will sink to the bottom

Yes, but ...

 

Professor Frances is probably the ultimate authority on diagnostic criteria for psychiatric diseases.

 

In a sense, he didn't just "write the textbook" - he wrote all of them.

 

The DSM-IV criteria are used by every psychiatrist on the planet.

He also hasn't meet Trump in person to properly diagnose him, and can only speculate about levels of distress and impairment.

 

Allen Frances, of all people, should know that he can't make a definitive diagnosis based solely on what he has seen of someones public persona on television.

 

Those saying Trump is mentally ill have insufficient information to reach their conclusion, likewise Allen Frances has insufficient information to reach his.

I get your point but I think you are wrong. It is unprofessional to conclude that someone is mentally ill on the basis of public statements and actions, but illness can be properly ruled out on the same basis. If Trump is seen not doing something that is necessary to make the diagnosis, then you can safely say he doesn't fit that particular profile. It is like technical fault-finding. Only one thing causes the problem, but many things can be ruled out.
