I'm trying to add an account on PayPal. Fake example:



1) The name that appears on my statements is in this format: MR A J SMITH

Would I put "MR A J" in the 'First Name' field?

2) What exactly am I supposed to put in the 'Bank Name'? "NZB" or "NZB Branchname"?

Bonus question: When things like PayPal and Newegg want your phone number, do they expect you to enter it with the greater area codes, like "64 022XXXXXXX" for a mobile or "64 09 XXXXXXX" for a landline?
