I note that the OP has a fairly high sync rate downstream, most likely even with no master filter and/or poor wiring. This suggests close proximity to the exchange / cabinet.
There have been modems that have had difficulty maintaining stable DSL connections on very short line lengths (cisco SRP527 was one) that required an updated firmware to resolve. Perhaps a similar issue here?
