Ok, the pic I posted clearly says iMessage in the reply line, as do all other iMessages I send, so surely that is how they are being sent

It seems that your iMessages may not be delivered every time, and the phone is failing back to SMS/MMS to send instead. This will happen if the iMessage can't be delivered in about 2(??) minutes. In the screenshot you posted, there is no "delivered" tag under each message that should be there once the receiving device gets it. If the receiver has read receipts turned on, then it will change to read once it has been.
In settings->messages you should be able to disable both SMS and MMS as well. Diabling MMS should stop it trying to send pics as MMS of not delivered as iMessage.
In settings->messages->send and receive as - make sure the correct phone number is selected, you may or may not want to also use your apple ID.
Most iMessage issues can be resolved by disabling and re-enabling. Had a problem quite a while ago that was resolved by disabling iMessage overnight (on all devices signed in using that Apple ID).
Ok, Iv'e checked on 3 or 4 contacts with iphones.
Each one has 'delivered' under the last text sent but no others which should explain why you didn't see it.
Are you saying I can send pictures via iMessage with MMS turned off?
I thought it had to be on
Turn MMS off - it is independent of iMessage.
If it's on, it will fail back to MMS if iMessage doesn't get through for whatever reason (receiver out of coverage etc.)
