I'm helping someone setup fast internet in a new place with only ADSL available (no VDSL).

What would be involved in getting two BigPipe ADSL connections setup using both twisted pairs of the phone line? (We will handle the load-balancing etc).

Is the total monthly cost $89*2 or more for a second line?

You simply need to order a 2nd connection. Installation costs could well be ins the $399 range as it will need to be a full install including wiring.

Great, am I correct in assuming the majority of that costs would cover terminating the second twisted pair inside the premises?

I.e. Assuming a second phone jack was already wired in place, would we be just looking at another standard install fee of $50 or is there more involved at the kerbside etc? And is all this handled by BigPipe?

Bigpipe will bring both the connections up to the house's ETP. You will be responsible for terminating it on the jackpoints. The fee should be $49x2 sign up fee and $79x2 monthly fee.
