Hey there i have a vodafone 720 and have brought the broadband just wondering what i have to do to connect it to the computer

I have plugged the cable in and gone to modem on the program it came with  tried to make a new connection and it comes up with user name password apn and phone numbers etc  i have tried doing what it said in someone elses post but it will not work 

Any ideas that can help me?
