After some recommendations on a good international SIM card provider. Countries that it will be used in:
US, UK, Italy, Finland, Norway.
Will probably use his Vodafone handset on roaming for incoming & use international SIM on out going & possibly data.

I have used a t-mobile UK sim card in New Zealand for a bit, I like it while your roaming you can recieve text messages for free,

Not too bad for outgoing calls (http://www.t-mobile.co.uk/shop/mobile-phones/price-plans/pay-as-you-go/international/) and data is pretty good while in the UK as well as well (£5 - 30 days of internet on your phone).

Roaming in places like italy on t-mobile is the same cost to text UK or overseas (10.2p per text)

Data is a bit expensive but for data a local sim for each country would probably be your best option.

Just in AA today and they have this new travelSIM - http://www.travelsim.co.nz/

Costs $50 for SIM and comes with NZ$15 credit.

Looks like the call rates in some countries aren't to bad.

Not sure about data - it says they have cheap rates but the don't actually state any values I can see.

It will still work out cheaper getting a local SIM but this would be easier.
