A "proper" custom setup option on install where I can choose to not install various components e.g cortana, paint, xbox stuff, etc.

I know LTSB/LTSC is an option, but it's not meant to be used on a desktop machine.

It's easy enough if you are using a proper deployment tool (e.g. MDT/SCCM), I remove unwanted apps offline during the WinPE phase with Michael Niehaus's script from here.
