nakedmolerat: looking at your mcnz, you dont work in emergency department.

look at this NZ emergency tutorial

my point: it is quite common to see patient with toothache in ED. it wont take more than 5 minutes to sort it out

That gave me quite a giggle.

Like Kevin I've also worked a lot in EDs (all doctors do in their junior years regardless of whatever they finally end up working in), and I can tell you that you'll wait hours before you get seen "in five minutes". There is always the very slim chance you arrive at the ED in a magical quiet time, but the odds of that occurring on a Friday night? Personally if I were feeling that lucky I'd buy a lotto ticket instead!

KevinL: Clearly there's little point continuing this discussion - remember, ED is for emergencies, and AA is for breakdowns.

Good night all, and good luck with the dentist OP!

spot on, i dont see this as a discussion since you miss my point from the start.

1. OP has toothache and he has organized to see a dentist within 24 hours. he made reasonable attempt to control the pain but asking for more help. he had been mixing OTC nsaids. giving further advise on online forum may not be a good idea after all since you are not able to ascertain correct dosage / assumptions etc.

2. my suggestion; if this is excruciatingly painful, attending emergency department could be an option to obtain stronger pain relief, local anaesthetic and advice. he could also attend 24 hr medical centre if that is an option (this is not available throughout nz, i dont know where OP is based).

3. it is not true to say that those waiting to see a doctor in the emergency room will not get any form of pain relief. triage nurse (usually a senior nurse) is smart enough to judge who should have pain relief while waiting.

4. my last reply re:AA is just trolling. you cant compare a broken indicator light with toothache / abscess.

As mentioned, I don't disagree with (most) of what you are saying.  Sam is based in South Auckland, has recently turned 18 and is certainly aware of the issues around attending ED for non-urgent issues (full credit to him).

Yes, the triage nurse is often senior and may request you be given pain relief - however, having worked in ED extensively, I can tell you explicitly that I would not prescribe anything more than simple analgesia (paracetamol and maybe an NSAID) to a patient sight unseen. It is medicolegally fraught with danger, and frankly poor practice.   Remember that you are still the last patient at the end of a very long queue, and that queue is potentially going to get longer as you sit there waiting.

Most major cities have a 24-hour accident and medical centre, and there are certainly many in the greater Auckland area - the issue of ED overcrowding is not a new one, and it is not going to improve until this attitude of "I'm just going to pop in to see the doctor quickly and then be on my way" is eradicated.  ED should be the third line of defense (after seeing your GP, and after seeing a 24 hr accident and medical doctor if available/applicable) - if you are going straight to ED, then it really ought to be for something that is potentially serious that requires immediate attention and/or potential hospital admission.

Murray, i think you misunderstood me. 5 mins to sort out (ie, analgesia, abx, local anaesthetic). not 5 mins waiting...

Mkay, was woken up by some little kid across the road screaming their head off abouT 10 minutes ago. I was fine for a bit then the pain just hit. It's really throbbing right now >< I don't have any of that clove oil mentioned earlier but I might go see if the dairy across the road has any. Might need to take out a loan for that because he is insanely overpriced.

You might be lucky if the dairy has Clove Oil, usually just seen it in chemists and supermarkets ... hang in there - only a couple of hours ....

This complete utter moron I booked with over the phone yesterday booked it for 1.30 instead, so I told the lady I want something sooner and the earliest they had was 1. >< so I've just taken two of the liquid nurofen tablets so they hopefully kick in soon because this is seriously so sore right now, I could barely brush my teeth without getting sharp shooting pains.

Seriously never going back to this place ever again after this is over. Not cool at all.

I could barely brush my teeth without getting sharp shooting pains.

Sounds like an abscess and the voltaren/panadol reduced it enough to get you through the night. Now its worn off it will be running riot in your system. (Murphy's Law.. an abscess always hits on a Friday night) lol

Its only another 2 hrs Sam so hang in there. Go to the dentist a little earlier and sit there moaning in reception...it might help. :)

Hugs Hon... theres nothing else you can do. If you have to drink anything.. drink through a straw. Any extremes (hot or cold) will set it off again.

Next time (and there will be a next time).. make the appt when the pain first starts. Dont forget to get your wisdom teeth checked out when you go today. They are technically useless to us.. a leftover from evolution. I remember coming back from Oz when I was your age.. fine when I got on the plane, screaming by the time it got back to NZ. They took me straight off the plane and into the hospital to have all 4 wisdom teeth out at once. Infection set in and it damn near killed me.

The dentist is your friend....hehe

Edited for clarity.

Yeh I absolutely regret leaving it this long. I thought towards the start of it that I might be able to leave it and itll go away on its own. But no of course it didn't haha. This couldn't have happened a month agocould it >< but atleast it happened on my mid semester break so I don't miss any classes.

Mkay so I'm back. Got seen way after 1, got 2 xrays done, and he was very nice I have to admit, probably because he's getting paid this time?

Anyway, he looked at the tooth and xrays and said the root or tooth is dead because a filling I've had in the past has done something or just didnt work, either way its ruined basically haha.

He said my options so far are:
Root Canal - $1,100 with possibility of extraction later if it doesn't work out/situation gets worse.
Extraction - $300+

He said the roots in this tooth are very tough, very deep and there are 3 of them as opposed to 1 in normal teeth apparently, so its looking like I will need to have the gum cut open and nerves cut off to get the tooth out which is the reason it costs so much.

He gave me some antibiotics; 9 Amoxicillin+clavulanic Ta 500+125mg (nov), which will last me until Tuesday morning if I do the correct dosages which I will. He said this should remove some swelling and help infection but said there was no abscess in the tooth though which is good but still not helping anything.

If this doesn't help remove pain then I'll either have to live with this pain, get treatment from him with either root canal or extraction, or go to Middlemore Hospital at 5am and wait in line to have it taken out for $40. ><

I laughed when he said $1,100 for the Root Canal. I'm a student, I have little money as it is, $1,100 can pay for my stationary for the rest of this year.

Anyway, I have the next few days to think over, and I have an xray picture I'll scan when I get home soon.

Ask any questions if you like. lol. I'm just gonna be taking these nurofen to keep it calm now too. >< FML
