Actually nothing to do with Yahoo! really, but social engineering and a bit of research. The person who accssed this account explained how he did it.

First he locked the account by trying to access it a few times with the wrong password. Then the reset system was easy to bypass - for each question he only had to use Wikipedia to find her birth date, and a bit of Google to find the answer to "How you first met your spouse?"... The answer was the high school name.

Nothing really "out of this world" and I would not blame Yahoo! if people use something that is easy to guess or find...
