I am trying to find software to do this... Anyone know of any good PHP scripts? Doesnt have to be PHP but I am unsure on what to search for (in terms of keywords)

So you would have a php script on the destination,  that can trace route traffic coming to the destination site?
and then have a form for the users to fill out with location, isp.

Can php do a trace or does it just see the ip address of the visitor.  So would you have to do a trace back to their IP address,
and that would just be testing you connection.
