Is it still best to set US region and language for Cortana?
It will be the one with features first, but I wonder if voice recognition and possibly localization features would be better with Australia?

I've not tried Australia, so can't comment on voice matching.  Cortana seems to work well with my voice on the US region/language.

I have mine set to Australia and have found the voice pickup to be on par with using the UK and quite a bit better than the US

It also seems to cancel your payment methods in your wallet app if you have any set up. Unsure if you ca add an NZ card to it if you are set to US, didn't try.

You can still download free apps when in the AUS store, but I tend to move back to the NZ store if I need to actually pay for an app.

I've followed the steps on this page (for Australia)
http://www.windowsphone.com/en-us/how-to/wp8/cortana/cortana-alpha 
and restarted when required - but I still don't have Cortana.

The article states that I need OS 8.10.14141.167, but my phone has 8.10.12393.890, with no further updates available (NZ 1020).

What am I doing wrong?

Sure can. Have been using my NZ CC with US Windows Phone and Windows 8.x store for a long time. Also works on Xbox.

I also have a 1020 which is set to Australia. Also has the same OS version as you.
I guess we are both stuck until another update comes available :-(
My phone is from Hong Kong and I got the Cyan update earlier than some others
here so was hoping I would get Cortana earlier too.. Still waiting though.

Australia is only available for those on 8.1 Update 1. (aka as Lumia Denim)

Interesting, my Ativ S has update 1 and after changing region and language to Aussie, it still wouldn't fire up saying my speech didn't match, but the only english speech setting I had were US and UK. 
Just trying it on UK now as I was having issues with the US voice recognition and it annoyed me having imperial units for temperature etc.

Have a 520 with region set to UK but there is no Cortana shown in the Settings menu. Phone is on 8.1 Cyan update..

Another issue with a region change is calling 0800 or 0900 numbers if you've enabled international assist. When calling one in NZ when your phone is set to another region it thinks you are attempting to dial overseas and may not work, or possibly incur charges, that is assuming it even works.
