Fiber ONT is in a concrete lined garage. The rest of the house is on the two floors above the garage. No easy way to run cable.

Should we get good results running a powerline adapter from the ONT directly, then having the router upstairs? This is for my parents, so their requirements are not huge, just netflix and ipads.

Technically no problem doing this however real world results are dependant on your home wiring.

See if you can get an open box unit to eval from your IT retailer (i.e. PB Tech) and check it works for you.

Powerline Adapters give very different results depending on, how old your wiring is and how far / how many circuits it has to cross to connect to the other point.

My brother in law only got like 30mbit, so eventually ran an ethernet cable. But I have seen people get 80mbit and sometimes in excess of 100mbit.

100mbit is a great connection speed. If you on a 100mbit plan then yes I think you will be fine. Each case is different. You need to try it and see. I would also use the opportunity to make sure your router is in a really good spot if its got built in wireless. Quite often its poor wireless that degrades peoples online experience.
