I heard it was per order, and the offer was recently removed.

My order has a dispatch date of "Dispatches by 7 August 2013" and it doesn't meantime Netflix anywhere at all, but the deal was there when I purchased.

Here is an excerpt regarding whether you need the external USB power outlet or not:

The power adapter is an optional requirement. In fact, most modern TV's with HDMI 1.4 or higher spec are supported. The Chromecast can draw power from the HDMI port it's plugged into. If your TV doesn't support that, there's also a USB cable and power adapter included in the box. I have a 2012 LG LED and a 2013 Lenovo 27" Monitor with HDMI input and the ports power it with no cable.

The device accesses the web independently, so I think you'll have to find another way.

Thanx for the info.  I alway thought that this link was only for ordering the Kindle from Amazon and not other stuff  but if I can order   CDs, movies,   hardware etc   via that link then I will from now on..

freitasm do you also get anything from amazon.co.uk if we just change the .com to .co.uk ??

I often order movies, books etc from .co.uk as they have better shipping rates a lot of the time.
