I started using Wideprotect https://itunes.apple.com/us/app/block-spam-calls-sms-mms/id1171024059?mt=8
As @michaelmurfy stated apps don't have direct API access to caller ID, so the app has to create a database of numbers which in turn apple callkit looks at for blocked numbers.
As of 11.3 you can block up to 30 million numbers now, prior to that it was 20, so my 3 wildcard blocks are fully covered now.
I was getting calls daily from the "UK" and "Canada", not answering them as I never get calls from either of those countries. The calls just go straight to voicemail, they accidentally left a message once and it was just Indians talking amongst themselves.
Since then I haven't had any more.
Here's what I'm blocking:
It's by far the least creepiest app, it doesn't need contacts permissions unless you want to whitelist them. It doesn't upload your contacts anywhere and their privacy policy isn't 600 pages long.
