I see 2D now have some good roaming packs (Globetrotter) for use in Canada, Ireland, UK and the US. I'm a little worried though that roaming in Japan will cost me $10 per MB!!! Outgoing calls are $3.39 min. When I next head off to Japan late September, is it likely Japan will have been added to those destinations? Spark and VF have roaming agreements in place, but 2D still have a long way to go.

I think that is a question for @SaltyNZ but I would be surprised if he can speak about future plans / changes before the information is made public

Usually better to look for a local SIM card in the country you are travelling to. Almost always a much cheaper option than roaming.
If visiting multiple countries you may also be able to pick up roaming cards overseas that bundle countries within the region together.

So much better to just get a local sim while over in Japan. When I was over there Oct last year I used Ninja Sim. Picked it up from the airport arrival lounge and worked pretty much off the bat. That is just one of the many options available to you.

I can't speak about possible future plans even if I know them, no. VoWifi will work while overseas and you will make and receive calls, SMS & MMS at your home rates. That doesn't help you for data, but then again, if you have VoWifi then you're already on Wifi by definition. If you need to use massive amounts of data while overseas, a local SIM is always the best way to go no matter which carrier you're on at home.

Also note that the phone has to have the 2degrees firmware on it to support VoWifi.
