I have no idea why she has had failed recordings.

I suspect if she records several programs on one night the recorder can't handle a setting for two consecutive programs on one channel and another one on another channel at the same time. I've had this happen and the problem is the overlap in times for recording consecutive programs. The recording starts one minute (or whatever time is set) beore the program start time and stops recording 5 mins (or whatever time is set) after end time. The only solution I've found for this scenario is to use timer programming for consecutive programs if recording another channel as well. Everybody seems to rely on the EPG function for recording these days forgetting that in the days of VCRs all recording was done by timer settings only. The EPG is only a guide and is not a guarantee that everything will work, nor is it always accurate. Timer recording is positive and won't fail.

There does seem to be room for a bit of improvement in the +1 channel recording logic. Perhaps if the EPG gave the +1 channels different IDs it might work better.

Recently I wanted to record a programme that had already started on One, and asked it to record on the One+1 channel via the EPG (and it indicated it was set to record both One and One+1).

However rather than recording the whole programme on One+1, it just recorded the remainder of the programme on One.

I guess the lesson is to record manually in these situations.

The failed recordings have not generally been around a time of another recording and have been programmes that have taped perfectly well every other week.  If there is a scheduling conflict on MySky it will tell you or at the very least partially record the programme.

Quite true. The recorder should bring up the conflict resolution dialogue.
The only other problem I've encountered with series link on and off all this year is sometimes in the middle of a series the link breaks and always seems to break between back to back series. Checking the prog/check shows no recording info for the next week but deleting and resetting the series link makes it work. This has to be a problem with the Freeviw EPG though and not the recorder. I think it happens after they do upgrades and changes to their system. I've got in the habit now of regularly checking with prog check to make sure the link hasn't broken for next episode. Has saved missed recordings a few times this year.

I've suggested she just check every day to make sure everything is ready to go but at 86 this is all too daunting for her.  Oh well at least we have managed to teach her how to view recorded programmes and she no longer has to muck around with all the video tapes she used to have.
