Afternoon all

I've trying to whip up these items, don't want to reinvent the wheel though.

I'm just after a generic change process form and or a release process form for changes to a network/software etc...

Anyone have something they can share, or know of a repository? Have searched a bit, either too simple or too complex!

Grab one online, delete unnecessary fields for your situation.  That should be a good starting point.  

I doubt anything anyone's using (and can freely share with you) fits your exact requirements...

Had a look online, mostly change management forms, couldn't find many release process or change process forms at all-  and the ones I found it wasn't worth mucking about with them.

Thought I'd ask here, no harm :)

Never know I guess.  

Might you be better off using some kind of online tracking system though?  Most bug/request trackers can be easily adapted for change management.
