go to the page in chrome
press the 3 dots in the top right corner
select add to home page
done
Open the site and go to the page you want in chrome.
Click the 3 dots to open Chrome's menu
Select "More tools" => "Create shortcut"
Give it a name then click "Create"
A short cut will be created on your desktop. Double clicking it should open Chrome and load the requested page.
This method should work on Windows, Linux and Mac.
Since this is "Android" and someone is asking here. I am guessing you want an icon on your phone/tablet.
From Chrome: Click the three dots on the top right, then choose "add to home screen", and choose what you want it to be called.
Then you can drag and drop the icon, or let the phone place it for you.
