I have been asked to provide private medical information about myself ahead of a medical appointment, by a large and reputable NZ private medical provider.

 

The information is required to be provided by answering questions etc, on a customised survey on the Survey Monkey web page.

 

I have only seen the first page/screen of questions without answering any of them. The request to collect the information indicates up to twenty minutes is required to complete the process, so I can only assume that questions beyond the first page seek considerable detail.

 

I have refused on the basis that I cannot see how a Survey Monkey website operating in a foreign jurisdiction can pass muster as far as NZ medical privacy considerations are concerned.

 

Am I being unreasonable?

 

Has anyone else in NZ been asked to provide personal medical information via a SurveyMonkey website and if so, what was their experience?

Ask them to scrape the info from the MMH breach.... see if they get the hint.

Ask them (The Medical provider) to provide you with a document that specifically shows "Survey monkey" has agreed to abide by the principles in  the "Health Information Privacy Code 2020"  as part of the NZ Privacy Act,

If they cannot provide you this, then tell them you will provide the information directly to the Medical provider,
but not via their proxy "Survey monkey" because the proxy has not shown that it is compliance with NZ laws

I have no special expertise in this area but I would not comply. I think this is completely unreasonable.

Just tell them that you are not computer literate and can only fill out a paper form.
