No, the downside of the ACC system is that, because no one gets sued, no one cares.

 

No - actually because of OSH regulations etc - you'll find that if you can phrase a statement and questions in a manner to pin liability on an individual, then things might happen:

Find name of most senior manager responsible - or if that's difficult, just go straight for the CEO.

Send email with made up but at least half-believable fabrication or even just potential future "incident".  Then state that "as I've let you know, then you can't deny responsibility on the basis of not having been informed of this potential safety issue"
