Lesson Learned, only use Volume-Camera-Menu-Power as an absolute last resort, i.e. when the phone simply will not start.
BrentR: You have to rename the file and drop the .dm off the end for that method to work.
TBH, it would probably be worth your while getting the handset sent to Nokia Care in Auckland so they can rebuild the firmware properly so you can actually use the Store application properly.
Have you thought about getting a public certificate for your Exchange so you don't continue to run into the issues you seem to experience across multiple platforms?

And also modify the file in Notepad++. The better way is to plug your phone into Ovi Suite and log in, then log into store.nokia.com and use the 'Send to phone' option. It can then send the installer to Ovi Suite which will download it and do any .dm conversion for you.
