My partner has her class 2.  She recently misplaced her license, and had it replaced at a VTNZ.

We just received it in the mail, but it only has her car class (1) and not her heavy truck one (2).

Why weren't both put on her license, surely it's just a replacement?

Yep someone cocked up like my replacement licence had DOB 1963 so I took it back and they would not change the DOB until I took in my birth cert or pass port so took in the ID and the replacement replacement still had DOB 1963

So you don't need medicals or anything to replace a class 2.  That was my thinking...

did she submit a medical certificate with her application, i have a class 2 and when i had to renew my license, to get the endorsement for class 2 on my new license i had to get a full medical and show it , i didnt so i dont have class 2 anymore but its not lost i just have to show a medical and it will be added.

Common sense is not as common as you think.

Shouldn't as it is a replacement licence not re-licence. But maybe that is a tidy source of revenue. They should have told her if that was the case though.
