Using the CSV data off the Chorus site, I've imported a list of all the exchanges in the country.  Does anyone know of where I can match an exchange to it's physical address?

Examples of the labels I have (there are 354 exchanges in total):

NB
WD
MPU
MIR
POP
PGA
LIN
GLF
CAM
BYM
COL
TRV
HP
ALY
MMA
HSD
CVE

I'm not sure that physical addresses are publicised in any way.  The user tehyitz from GPForums converted the shapefiles from Chorus' website to Google Earth's KML format, but many of the co-ordinates are inaccurate by a considerable margin, so exact physical addresses could probably only be surmised in a minority of cases.  The exchange boundaries are probably much more accurate than actual exchange locations.  You can find those files here.

A wiki page was also started through GPForums several years ago, where people who knew the exact location of one or more exchanges could add them to the list.  There are only a handful of locations on the list though, and the page hasn't been updated in awhile (still has smaps links, and zoomin links need updating to new URL format).  They are however pretty accurate for the most part.  You can find the wiki page here.

For me, the main value of knowing the exchange locations has always been to get a rough idea of expected ADSL performance at a given address.  Telecom Wholesale's Service Availability Tool has now satisfied that to some extent.
