Hey guys.

How many people have done this and managed to achieve this as a permanent fix?

I have tried and on my previous modem, I had all the correct ports open however since my modem crashed and I cant remember all the exact ports, when my partners PS4 is turned on first, I will have a moderate NAT type.

Any suggestions here? Yes I have assigned my PS4 a static IP address also.

DO NOT put your PS4 into a DMZ. This is a terribly bad security practise. Your router should have UPNP or NAT-PNP that can be enabled which will allow your PS4 to go into an "Open/Moderate NAT". You shouldn't ever have to manually port forward to a PS4 or a Xbox and if you are then you're doing it wrong.
