I bought my first ebook yesterday on my new kindle using the 2nd account method, but Amazon has caught me out and I have received THE letter! If I now deregister my kindle on the 2nd (fake) account, and re-register on the legitimate one, will I lose the book on my kindle (which I haven't read yet!)? The book, as at yesterday, hadn't appeared in My Library on Amazon either.
If the book is on your Kindle, you can change registration OK (I did this). You can copy the files off the kindle anyway to back them up (they are in the Documents folder).
