I just bought a brand new apple watch series 7 from PBTech, and the battery is dead out of the box.   Normally my apple products have some battery when I first get them.

Should I be worried?  I don't want problems down the road with a battery that has issues.

I'm trying to charge now so have not yet had a chance to look at the battery health (assuming that would tell me something)

That's super unusual. All of my Apple Watches have always had at least 50% battery on unboxing.

I'd say the only thing to do at this stage is what you are already doing.

I received one from Spark a few months ago in the same state.

It appears not to have suffered from it right now, however it did take a good number of hours to get it fully charged the first time.

I also received a dead Apple Watch from Spark a while back, had no problems with it once it was charged. Also had dead AirPods Max from Noel Leeming but they were fine too.

I would contact PB Tech just so there is a record, or make a record of it in some way, so if there are issues you can use that to claim a DOA replacement.

Wondering if the current Supply chain disruptions are resulting in stock having to sit in warehouses or on the water much longer than the pre pandemic factory to consumer journey..?

I have ordered products directly from Apple during the apocalypse and they have all turned up quickly with about 50% charge in the battery.

In this example, the product was purchased from a retail outlet and it's possible that it has been sitting in stock for a long time if the particular variant (colour/size/cellular capability) hasn't been in demand. It's not ideal for lithium ion batteries to sit under 20% charge state for a long period of time, but it's unlikely that it will cause any problems in the long term.
