I'm on 4.2.1 on an iPhone 4. Just tested and a new non-recurring alarm didn't go off for me

Yep, verified here - iPh4 4.2.1

As reported elsewhere, set it up as a repeating alarm and it works fine.

I'm pretty sure they will be well aware of the issue by now.

It doesn't mean it's going to be fixed before February of whatever year of your choice... You get what you pay for.

Oh, wait. It's not cheap.

Why not use an actual alarm clock? Surely it would be louder than an iPhone, and wouldn't have the "glitches" the iPhone as

My cellphone has been my alarm clock for more than 10 years. Why should I accept that a 'smart' phone is dumber than my old nokia 5110?

Cellphone lives on the bed-side table, isn't able to be tucked under a blanket or pillow like my wristwatch, and is 'always on' anyway.

Fortunately I have my R101 to lean on for the time being.
