I am looking for a VDSL2 modem that supports the following ONLY - can some one direct me to a product if you no of one.

WAN - Obviously Supports VDSL2 with all the profiles, also a WAN connection for the ONT in the future when UFB eventually gets here.
Minimum above this would be one Gigabit port, 2 extra would be preferred.

Does anyone know of a modem with just these specs? I don't want or need wireless and/or media services or any of the usual ad ons that go with them.

If you would let me know it would be appreciated as I prefer to split my boxes to - modem - switch - wireless as opposed to having an integrated all in one device that strains under load.

Redundant.

You won't need a modem. You will need a router and switch. OR you could have a combined router/switch.

EDIT 
I like separates too.
I'm on cable and use  modem > router > switch > wireless access point
