In bulk, probably costs Apple less than 50 cents.
Actually 15 cent - 17 cents is what they are quoting on the custom form .
Five days ago I ordered (online) a replacement duckhead adapter for my iPad 2. There was absolutely nothing wrong with the old one.
It arrived today (adapter only) - ridiculously overpackaged in an A3-sized TNT bag from Sydney - containing 4 x A4 pages of documentation, and another oversized TNT bag to courier the old adapter back to Sydney.
What would happen if I failed to return the old adapter? This seems like a remarkably wasteful process.

Same here - I ordered four, all with different serial numbers, and they're all still saying processing.
Wonder how long it takes? I suppose a lot of people are claiming on it, so may take a little while to process.

So I now have 9 of my 11 ready to go back... but there are no instructions as to which bit of paper goes where. Do I keep any of them or send the lot back in the clear pocket on the outside of the bag?

I notice a number of these plugs on Trademe.
I contacted one seller, who days later is still trying to sell these plugs.
I wonder what the consequences of knowingly selling items under safety recall is.

So they are selling the new plug adapter? If they are selling the old plugs, then they could apply to have the adapters switched free anyway. Also some being sold may not be orginal apple ones.

I sent them all back, in the clear pocket - think they are for customs.
They don't make it easy to get it back to them. Apparently you can do it online on teh TNT website, but their websites is poor, and it looks like you have to first register, and then they have to manually approve it. Why bother. Or you can phone, but it is closed today, and no automated way to enter the number for pickup. You would think in this day and age, it would be an automated system

I wish they would do something about the same problem prior to this charger they got sued for, with the rubber fraying near the L pin to laptop. Replaced 3 times now. $600, nearly worth the price of the laptop. Fast finding PC alternatives to what I need on Apple.
