Having two independent systems fail is rare, 

 

My money is on no one noticing that the primary system had failed, and the backup , which is usually spec'd for periods of short use failed after having to run for an extended period to cover the primary failure...

It’s unlikely that’s the case. Generally in a small server room, like a hospital, cooling is run on a duty/standby with two ACs of the same spec. Once a week the duty and standby units swap to give them both even run hours.

There are other ways to do it, like using chilled water with a refrigerant unit as standby but that’s not the way it would normally be done.
